Returning Patients
Welcome back to Upstate Vet! Please complete the relevant forms below prior to your upcoming visit. When bringing your pet for a follow-up visit, please bring any medications that your pet is currently taking.

Medication Refills
You can now request a refill for your pet’s prescription online by submitting the form below. Please allow one full business day to process your request. If there are any problems with refilling the prescription, we will contact you via phone. Medications can be picked up during our normal specialty business hours.
Returning Oncology Patients
If you are returning to our oncology department, we ask that you complete the following oncology history form. You may bring it to your appointment, submit the completed form via email, or fax to us.
Returning Surgery Patients
If you are returning for your pet to have surgery we ask that you complete the following presurgical patient medication form. You may bring it to your appointment, submit the completed form via email, or fax to us.
